IMPORTANT: PLEASE READ CAREFULLY BEFORE REGISTERING

By registering for and attending the Cell Surgical Conference (“CSC”), you acknowledge that you have read, understood, and agree to be bound by the following Terms & Conditions.

Registration & Cancellation Policy

Due to the limited capacity of our educational programs, hands-on workshops, live demonstrations, faculty commitments, and venue obligations, the following cancellation policy applies:

Cancellation Requests

All cancellation requests must be submitted in writing to CSC/REVIVE Institute & Training.

• 90 or more days prior to the conference: Registration fees will be refunded less a $250 administrative processing fee.
• 31–89 days prior to the conference: Registration fees are non-refundable; however, registrants may transfer their registration to another qualified attendee or receive a credit toward a future CSC event, valid for twelve (12) months.
• 30 days or less prior to the conference: No refunds, credits, or cancellations will be granted.
• No-shows: No refunds, credits, or transfers will be provided.

Registration Transfers

Registration transfers may be approved at the sole discretion of CSC and REVIVE Institute & Training. Transfer requests must be submitted in writing prior to the event.

Conference Cancellation or Rescheduling

CSC reserves the right to modify, postpone, reschedule, substitute faculty, adjust programming, or cancel the conference due to circumstances beyond its reasonable control. Should CSC cancel the conference, attendees will receive either:

• A full refund of registration fees paid; or
• A credit toward a future CSC event.

CSC, REVIVE Institute & Training, Cell Surgical Network, faculty, sponsors, and affiliates are not responsible for airfare, hotel accommodations, transportation expenses, lost income, or any other travel-related costs incurred by attendees.

Educational Purpose Disclaimer

The Cell Surgical Conference is an educational event intended exclusively for healthcare professionals and qualified practitioners. All presentations, lectures, demonstrations, workshops, case studies, panel discussions, educational materials, and faculty commentary are provided solely for educational and informational purposes. Nothing presented at CSC should be construed as medical advice, legal advice, regulatory advice, reimbursement advice, compliance guidance, business advice, or a recommendation for patient treatment. Attendees are solely responsible for the clinical decisions they make within their own practices.

Off-Label, Investigational & Emerging Therapies Disclosure

Attendees acknowledge that conference faculty, presenters, sponsors, and exhibitors may discuss therapies, devices, biologics, pharmaceuticals, procedures, protocols, and clinical applications that may be off-label, investigational, experimental, emerging, or not FDA-approved for the specific uses discussed.

Discussion of any therapy, device, protocol, procedure, biologic, pharmaceutical, or clinical application does not constitute endorsement, recommendation, promotion, certification, or approval by CSC, REVIVE Institute & Training, Cell Surgical Network, faculty, sponsors, exhibitors, or affiliates.

Attendees are solely responsible for ensuring compliance with all applicable federal, state, local, licensing, accreditation, professional, and regulatory requirements governing

Hands-On Training & Live Demonstration Disclaimer

CSC may include hands-on workshops, procedural demonstrations, live patient demonstrations, ultrasound-guided procedures, biologic processing techniques, injection protocols, regenerative medicine applications, and other educational training activities.

Participation in any workshop, demonstration, or training session:

• Does not constitute certification.
• Does not establish competency.
• Does not authorize the performance of any procedure.
• Does not establish a standard of care.
• Does not create any supervisory or mentoring relationship between faculty and attendees.

Attendees acknowledge that medical procedures carry inherent risks and that clinical outcomes vary based on patient selection, practitioner training, technique, and numerous other factors.

No Guarantee of Outcomes

CSC, REVIVE Institute & Training, Cell Surgical Network, faculty, sponsors, exhibitors, and affiliates make no representations or warranties regarding:

• Clinical outcomes
• Treatment efficacy
• Patient satisfaction
• Regulatory status
• Reimbursement potential
• Financial performance
• Practice profitability
• Commercial success

Any patient cases, testimonials, treatment results, success stories, photographs, videos, or clinical outcomes presented during the conference are illustrative only and should not be interpreted as guarantees of future results.

No Physician-Patient Relationship

Attendance at CSC, participation in conference activities, interaction with faculty, receipt of educational materials, or participation in workshops does not establish a physician-patient relationship, consulting relationship, supervisory relationship, mentorship relationship, or any professional duty owed by faculty to attendees.

Attendees remain solely responsible for all medical decisions, treatments, and services provided within their own practices.

Assumption of Risk & Release of Liability

Attendees voluntarily participate in all conference activities and assume all risks associated with attendance.

To the fullest extent permitted by law, attendees release and hold harmless:

• Speakers
• Sponsors
• Exhibitors
• Contractors
• Employees
• Officers
• Directors
• Affiliates

from any and all claims, liabilities, damages, injuries, losses, costs, or expenses arising out of or related to attendance, participation, reliance upon educational content, or implementation of information presented during the conference.

Photography, Video & Media Release

By attending CSC, participants consent to being photographed, filmed, recorded, livestreamed, or otherwise captured in conference-related media.

CSC, REVIVE Institute & Training, and Cell Surgical Network may use such photographs, recordings, images, audio, video, and likenesses for educational, promotional, marketing, advertising, social media, and future conference purposes without compensation or additional consent.

Agreement

By checking the box below and completing registration, I confirm that I have read, understood, and agree to all of the Cell Surgical Conference Registration Terms & Conditions above, including the cancellation and refund policy, the educational purpose and off-label therapy disclosures, the hands-on training disclaimer, the no guarantee of outcomes and no physician-patient relationship provisions, the assumption of risk and release of liability, and the photography, video, and media release.

I understand that portions of the conference may include discussion of off-label, investigational, or emerging therapies, and that educational content and demonstrations do not constitute certification, medical advice, legal advice, or regulatory guidance.

I voluntarily assume all risks associated with attendance and participation.
